THE COURT:

It is ordered that the opinion filed herein on June 10, 2014, be modified as follows: 1. On page 21, the first line of first full paragraph, the words “Blue Cross” are deleted and replaced with the words “Blue Shield.” 2. On page 23, line six of the second full paragraph, the word “DMHC” is deleted and replaced with the words “Department of Insurance.” There is no change in the judgment. The petition for rehearing is denied.
