I. BACKGROUND

A. Establishment of Dependency Proceedings On June 17, 2013, the San Joaquin County Human Services Agency (San Joaquin) filed a dependency petition with respect to the three minors who are the subject of these proceedings, alleging that the boys were at risk of harm due to filthy and hazardous conditions in the family home, mother’s ongoing mental health issues, her past drug use, and the sexual abuse of C.C. by Joshua C., the alleged father of S.H. and C.H. Specifically, in May 2013—after mother called 911 stating that she was going into labor with C.H.—the Stockton Police Department responded to mother’s home and discovered numerous unsafe conditions, including: boxes stacked up all around the front room; broken and dirty couches; the smell of rotting food, garbage, and fecal matter; kitchen counters full of old, moldy food; cockroaches and other bugs in the oven; garbage and soiled diapers on the kitchen floor; and bottles of pesticide and a knife lying within reach of the minors. Mother had videos of various traps and poison plugs placed around her home and “thousands of cockroaches scattering on the wall.” C.C. reported that cockroaches crawled in his ears…
