Plaintiff Linda Arnold (“Plaintiff” or “Arnold”) seeks benefits under a an employee health and welfare benefits plan (“Plan”) sponsored by the Health Care Employees/Employer Dental & Medical Trust (“Plan Sponsor”). The Plan is fully insured under a contract between the Plan Sponsor and United Healthcare Insurance Company (“United” or “Defendant”), and funded by the payment of premiums.
