17 INTRODUCTION Plaintiff Joni Winburn, proceeding through counsel, initiated this action by filing a complaint on October 4, 2019 against defendant Hartford Life and Accident Insurance Company. (Doc. No. 1.) Therein, based upon her allegation that defendants terminated her long-term disability benefits and denied her internal appeals of the termination of those benefits, plaintiff asserted causes of action for breach of contract and bad faith.
